Job Title: Wellbeing Connector

Location:  Cardiff
Hours: Various hours available between 15 – 37.5 hours per week, please state your preference on application.  Please note we may not be able to offer the preferred hours but will endeavour to match your preference as closely as possible.
Salary: £23,964 per annum (pro rata)
Contract: Fixed term until 31st January 2026 (with potential extension depending on funding)

We are seeking 3x Wellbeing Connectors to join the Yourspace team. Yourspace is a wellbeing service, working in partnership with the North, West and South West Cardiff Primary Care Clusters to support patients from local GP practices and the wider community to improve health and wellbeing through the provision of 1:1 and group-based support; community outreach activities and collaborative work to identify and fill gaps in local provision.

The role of Wellbeing Connector will have responsibility for supporting a caseload of patients to improve their health and wellbeing, through listening and helping patients to identify their own assets, needs and goals; linking to suitable groups, services and activities within the community; and supporting and encouraging patients to move towards their goals.  The role will also involve building links with the local community and service providers, collecting information on local services and gaps in provision and supporting outreach work.

Job Title: Community Health Development Worker

Location:  Cardiff
Hours: 26.25
Salary: £27,307 per annum (pro rata)
Contract:
Fixed-term, end date January 31st 2026 (with potential extension depending on funding)

We are seeking a Community Health Development Worker to join the Yourspace team.  Yourspace is a wellbeing service, working in partnership with the Cardiff North, Cardiff West and Cardiff South West Primary Care Clusters to support patients from local GP practices and the wider community to improve health and wellbeing through the provision of 1:1 and group-based support; community outreach activities and collaborative work to identify and fill gaps in local provision.

The role of Community Health Development Worker will take a lead role in building partnerships with local community organisations and service providers; working collaboratively to identify and fill gaps in local health and wellbeing provision; and supporting the development of a mix of volunteer-led and service-provider led groups, courses, activities and services to serve the Cluster areas.  This role will focus on the Cardiff North and Cardiff West Cluster areas.

Job Title: Young People’s Wellbeing Connector

Location:  Cardiff
Salary: £23,964 per annum (pro rata)

Fixed term end date January 31st 2026 (with potential extension depending on funding)

We have two posts available, with hours and geographical focus as follows
Hours: 30       Focus: Cardiff North Primary Care Cluster area
Hours: 15       Focus: Cardiff South West Primary Care Cluster area

Job Title: Postcode Gardener – Ely and Caerau

Location:  Ely and Caerau, Cardiff
Hours: 7.5 hours per week,
Working Days / Hours: TBC
Salary: £27,307 per annum (pro rata)

Contract: Fixed term 30th Sept 2026.
Closing date: Midnight, Sunday 24th November 2024

ACE is looking for someone with a love of gardening and community to become the Postcode Gardner in Ely and Caerau. This is a great opportunity to lead a programme to help residents improve their health and wellbeing and create greener neighbourhoods.

The role aims to inspire people to get together, get planting and keep growing, focusing on activities that both encourage urban wildlife and build community wellbeing.
