We are recruiting:

We are recruiting for an additional Wellbeing Connector, Community Health Development Workers x 2 and Young People’s Wellbeing Connectors x 2 to join the Yourspace team, please see details of the post below and in the attached job pack.

Position: Wellbeing Connector

To have responsibility for supporting a caseload of patients to improve their health and wellbeing, through listening and helping patients to identify their own assets, needs and goals; linking to suitable groups, services and activities within the community; and supporting and encouraging patients to move towards their goals.  The role will also involve building links with the local community and service providers, collecting information on local services and gaps in provision and supporting outreach work.

Position: Wellbeing Connector
Location: Cardiff
Hours: 30 hours / week
Salary: ACE Grade 2 – £23,964 per annum (pro rata)
Contract: Fixed term until 31st January 2026 (with potential extension dependent on funding)

Position: Community Health Development Workers (x2)

To take a lead role in building partnerships with local community organisations and service providers; working collaboratively to identify and fill gaps in local health and wellbeing provision; and supporting the development of a mix of volunteer-led and service-provider led groups, courses, activities and services to serve the Cluster area(s).

Hours: 22.5 Focus: Canton and Riverside

Hours: 15 Focus: Cardiff West Primary Care Cluster area

Salary: £27,307 per annum (pro rata)

Contract: End of January 2026 (with potential extension depending on funding)

Position: Young People’s Wellbeing Connectors (x2)

The role is primarily to work with young people aged 13-19, but may also involve work with their parents / carers.  The post-holder will have responsibility for supporting a caseload of young people to improve their health and wellbeing, through listening and helping young people to identify their own assets, needs and goals; linking to suitable groups, services and activities within the community; and supporting and encouraging young people to move towards their goals.

Hours: 30 Focus: Cardiff North Primary Care Cluster area

Hours: 15 Focus: Cardiff South West Primary Care Cluster area

Salary: £23,964 per annum (pro rata)

Contract: Fixed term until 31st January 2026 (with potential extension dependent on funding)
